What should be included when submitting a claim?

Claim processing time:
Up to 5 business days from the moment all required information is received.

Limitation Period:
In accordance with Amendment No. 11 to the Insurance Contract Law 1981, which came into force on November 25, 2020 and applies to policies purchased as of that date, the limitation period for submitting an insurance claim is five (5) years from the date of the occurrence of the insurance event, and in the case of minors under the age of 18, Five (5) years from the day they turned 18. The limitation period for insurance claims for policies purchased before November 25, 2020 is 36 months from the date the insurance event occurred. Please note: filing a claim does not stop the limitation period.
If additional information or additional documents are required to in order for the claim to be adjudicated, they must be submitted as early as possible and before the expiration of the limitation period detailed above. Failure to submit additional information required to adjudicate the claim within the limitation period may cause rejection of the claim due to the expiration of its limitation period. It should be emphasized and noted that filing a lawsuit in court as a result of the insurance event, stops the limitation period and in this case the provisions of the Israeli law will regarding limitation period shall apply.
In accordance with the Limitation period Law (Amendment No. 8 – Iron Swords) 2024, the period between October 7, 2023 or the date of the insurance event, whichever is later, and April 6, 2024 will not be counted.
Right to Appeal:
If you are not satisfied with the Company’s decision regarding your claim, you have the right to request a review by contacting any of the following:

Medical services in the USA are provided through UnitedHealthcare (UHC) under the Choice Plus plan.
When seeking medical care, please present your insurance details to the medical provider.
Your insurance card is always available via the app.
After your visit, the medical provider submits the claim directly to UHC, so no action is required on your part.
